Changes to the Terms of this Notice

Complaints

  • Complaints about this Notice of Privacy Practices or how this practice handles your health information should be directed to us via the contact information on our website. If you prefer, you can discuss your complaint in person or by phone.


If you are not satisfied with the manner in which our company handles a complaint, you may submit a formal complaint to U.S Department of Health and Human Services.  You will not be penalized in any way for filing a complaint.
